Why this applies to Bosentan

Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the gist for Bosentan (Tracleer) and red clover. Your liver clears bosentan partly through an enzyme called CYP2C9. In lab dishes, red clover's plant estrogens look like they might slow that enzyme down a little, which in theory could let bosentan build up slightly and bring on more of its usual effects, like flushing, headache, swelling in the legs, or a drop in blood pressure.

But here's the reassuring part: a real human study found that red clover didn't actually change how the body handled a CYP2C9 medication. So this is mostly a theoretical, test-tube concern rated minor and unlikely. Since bosentan is a serious medication for your lungs, it's still worth a quick chat with your pharmacist or doctor before adding red clover, but you don't need to panic.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
