Cariprazine Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist for Vraylar (cariprazine) and red clover. Your liver uses an enzyme called CYP3A4 to clear cariprazine from your body. In a test tube, red clover's plant estrogens looked like they might slow that enzyme down, which in theory could let cariprazine build up a bit and possibly bump up side effects like drowsiness, restlessness, dizziness, or tremor.

The good news is that this is mostly a theoretical worry. When researchers actually gave people standardized red clover capsules and checked a similar CYP3A4 drug, they saw no real change. So a meaningful interaction here is unlikely. Still, since cariprazine is a serious medicine, it's smart to mention any red clover use to your pharmacist or doctor before adding it.

What Is Cariprazine Used For?

Brand name: Vraylar

Vraylar (cariprazine) is approved to treat schizophrenia in adults and teens 13 and older, and to treat acute manic or mixed episodes associated with bipolar I disorder in adults and children 10 and older. In adults, it is also approved for depressive episodes associated with bipolar I disorder (bipolar depression).

Additionally, Vraylar is approved as an add-on therapy alongside antidepressants for adults with major depressive disorder (MDD) who need extra help beyond their current antidepressant alone.
