Why this applies to Nintedanib Esylate

Nintedanib Esylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist for Ofev (nintedanib). Your body partly uses an enzyme called CYP3A4 to handle this medicine. In a test tube, red clover looked like it might slow that enzyme down, which in theory could let a little more nintedanib build up. If that happened, you might notice more of its usual side effects, mainly diarrhea, nausea, stomach upset, or changes in liver tests.

The reassuring part: this is only a theoretical concern. When researchers actually gave people standardized red clover capsules and checked a CYP3A4 test drug, nothing meaningful changed. So a real-world problem is unlikely. Still, since nintedanib is a serious lung medication, please run any red clover supplement past your pharmacist or doctor first.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the gist with red clover and Ofev (nintedanib). Nintedanib itself can mildly raise bleeding risk, and red clover has been flagged in lab studies as possibly thinning the blood a little, mostly because of a compound called genistein. The worry is that stacking the two could add up to easier bruising or bleeding.

That said, the real-world evidence here is weak. When researchers actually measured clotting in people taking red clover for up to two years, they saw no meaningful change. So this is more of a theoretical caution than something most folks will notice. Still, since nintedanib already nudges bleeding risk on its own, it's worth a quick chat with your pharmacist or doctor before adding red clover, especially if you notice unusual bruising, nosebleeds, or blood in stool.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.

About Nintedanib

What Is Nintedanib Used For?

Brand name: Ofev

Nintedanib capsules are approved for adults with idiopathic pulmonary fibrosis (IPF) — a chronic, progressive scarring of the lungs — and for adults with chronic fibrosing interstitial lung diseases (ILDs) with a progressive phenotype, meaning fibrosing lung diseases that continue to worsen over time despite treatment.

Nintedanib capsules (including Ofev) are also indicated to slow the rate of decline in lung function in adults with systemic sclerosis-associated interstitial lung disease (SSc-ILD), a lung complication of the autoimmune condition systemic sclerosis (scleroderma). Note that the Nintedanib esylate capsule product is approved for IPF and chronic fibrosing ILDs with a progressive phenotype, but SSc-ILD approval is carried by Ofev and other nintedanib capsule products — always confirm with your prescriber which product you have been prescribed.
