Pazopanib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist for folks on pazopanib (Votrient). Your body clears this cancer medicine using a liver enzyme called CYP3A4. In a test tube, red clover's plant compounds (isoflavones) look like they might slow that enzyme down a little. If that happened in real life, pazopanib could build up and bring on stronger side effects, things like high blood pressure, diarrhea, nausea, fatigue, or changes in liver tests.

The reassuring part: this concern is mostly theoretical. A small human study using a similar CYP3A4 substrate found no real change, so the actual risk looks low. Still, pazopanib is a serious medication, so I'd run any red clover supplement past your oncology team or pharmacist before starting it.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the gist for Votrient (pazopanib) and red clover. The concern on file is about a possible extra blood-thinning or anti-clotting effect from red clover. In a test tube, one of its compounds (genistein) showed mild antiplatelet activity, and people used to worry about its coumarin content. But when researchers actually studied red clover in postmenopausal women, even at supplement doses for up to two years, they saw no real change in clotting or bleeding measures.

So in practice this is a minor, unlikely interaction backed by fairly weak evidence. Pazopanib itself can raise bleeding risk and affect blood pressure, so it makes sense to keep an eye out for things like easy bruising or unusual bleeding. Before adding red clover, just run it by your pharmacist or oncologist, since hormone-like supplements deserve a quick check during cancer treatment.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
