Pitolisant Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version on combining red clover with pitolisant (Wakix). Your body clears pitolisant partly through a liver enzyme called CYP3A4. In a test tube, red clover looks like it could slow that enzyme down a bit, which in theory might let pitolisant build up and nudge up side effects like headache, trouble sleeping at night, nausea, or anxiety.

The good news is that this is mostly theoretical. When researchers actually gave people standardized red clover and tested a similar CYP3A4 drug, they saw no real change. So a meaningful interaction here is unlikely. Pitolisant isn't a prodrug, so it doesn't flip the other way. Still, since pitolisant has its own heart-rhythm cautions, it's worth a quick check with your pharmacist or doctor before adding red clover.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. What Is Pitolisant Used For?

Brand name: Wakix

Wakix (pitolisant) is used to treat two hallmark problems of narcolepsy: excessive daytime sleepiness (feeling overwhelmingly tired and unable to stay awake during the day) and cataplexy (sudden episodes of muscle weakness or loss of muscle control, often triggered by emotion). It is approved for adults and children 6 years of age and older.

Both uses are specific to Wakix tablets taken by mouth. The drug is not approved for sleepiness or cataplexy caused by conditions other than narcolepsy.
