Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase the levels and clinical effects of drugs metabolized by CYP2C19.
In vitro evidence suggests that red clover weakly inhibits CYP2C19. This interaction has not been reported in humans.

Here's something interesting about proguanil: it's actually a prodrug. Your body uses an enzyme called CYP2C19 to convert it into its active form, cycloguanil, which is the part that fights the malaria parasite. So when we talk about something slowing that enzyme down, it works the opposite of what you might expect.

Red clover has shown, in lab-dish (test tube) studies only, that it might weakly block CYP2C19. In theory, that could mean a little less proguanil gets turned into its active form, which could make it slightly less effective at preventing malaria. That said, this has never been seen in real people, the effect is rated minor and unlikely, and good malaria protection matters. If you take proguanil for travel, just check with your pharmacist or doctor before adding red clover.
