Ramelteon falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist with Rozerem (ramelteon) and red clover. Your liver uses an enzyme called CYP1A2 to clear ramelteon out of your system. In a test tube, red clover's plant compounds (isoflavones) can slow that enzyme down a bit. If that happened in your body, ramelteon levels could nudge upward, which might mean a little extra drowsiness, grogginess, or feeling groggy the next morning.
The reassuring part: this is mostly lab-dish theory. A small human study using standardized red clover showed no real change in how the body handled a similar CYP1A2 drug. So a meaningful interaction here is unlikely. Still, since both can make you sleepy, it's worth a quick word with your pharmacist or doctor before combining them.

In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the short version: this pairing is very low on my worry list. The concern on paper is that red clover's plant estrogens (isoflavones) might slow down an enzyme called CYP2C9, which helps your body clear certain medicines. If that happened, a CYP2C9 drug could build up a bit and hit harder. For Ramelteon, that would mean more drowsiness or grogginess, maybe feeling foggy or sleepy the next morning.
But the evidence here is weak. The idea comes from lab (test-tube) studies, and a small human study showed red clover did not actually change how the body handled a CYP2C9 test drug. So a real-world problem is unlikely. Ramelteon isn't a prodrug, so this wouldn't weaken it. Still, it's worth a quick chat with your pharmacist or doctor before combining them.

However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version: your body clears ramelteon using a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down, which in theory could let ramelteon build up a bit and make you feel groggier or sleepier than expected the next morning. So far, though, this is mostly a paper concern.
When researchers actually tested red clover isoflavones in people, using a similar CYP3A4 medicine, they saw no real change in drug levels. The interaction is rated minor and unlikely, and ramelteon is not a prodrug, so it doesn't flip the direction. I wouldn't lose sleep over it, but it's always smart to mention any supplement to your pharmacist or doctor first.
