Selumetinib Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist: your child's selumetinib (Koselugo) is cleared from the body in part by a liver enzyme called CYP3A4. In lab dishes, red clover looked like it might slow that enzyme down a bit, which in theory could let selumetinib build up and make side effects like diarrhea, nausea, rash, mouth sores, or tiredness more likely. But that's only the theory.

In a real human study, red clover standardized to its isoflavones did not actually change the levels of a CYP3A4 test drug. So the concern is mostly on paper, and a true problem is considered unlikely. Still, selumetinib is a serious cancer-type medicine with careful dosing, so I wouldn't add red clover without first checking with your child's oncology team or pharmacist.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the honest picture with red clover and selumetinib (Koselugo). The concern is purely theoretical: some lab studies hinted that red clover's plant compounds might thin the blood a little, but actual human studies (including one that ran two years) found no real effect on clotting. So the risk of any bleeding-type interaction is considered minor and unlikely.

That said, selumetinib itself can sometimes cause bleeding or bruising as a side effect, so stacking anything that even might add to that is worth a quick mention. Because this medicine treats a serious condition in children, please don't add red clover on your own. Run it past your child's oncology team or pharmacist first so they can keep an eye on things.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
