Sertraline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version: in the lab, red clover looked like it might slow down CYP3A4, one of the liver pathways that helps clear sertraline (Zoloft). If that happened in real life, sertraline could build up a bit and you might notice more of its usual side effects, like nausea, jitteriness, trouble sleeping, or an upset stomach. Sertraline is not a prodrug, so blocking that enzyme would tend to raise its level, not lower its effect.

The reassuring part is that when researchers actually tested red clover in people, it did not meaningfully change levels of a CYP3A4 test drug. So this concern is mostly theoretical and rated minor and unlikely. Still, since both touch hormones and mood, it's worth a quick check with your pharmacist or doctor before combining them.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.

What Is Sertraline Used For?

Brand name: Zoloft

Sertraline treats a range of mental health conditions. All formulations are approved for major depressive disorder (MDD) in adults and obsessive-compulsive disorder (OCD). The oral solution (including Zoloft oral solution) and several tablet formulations are also approved for panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).

For OCD specifically, the capsule formulations and some tablet products are approved for use in adults and children aged 6 and older. Sertraline hydrochloride tablets (plain, not film-coated) are approved for OCD in pediatric patients as well. However, sertraline is not approved for major depressive disorder in children — only in adults.
