Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the short version: this is a very weak, mostly theoretical concern. In lab dishes, red clover can slow down an enzyme called CYP1A2, and the idea is that blocking that enzyme could let certain drugs build up. But when researchers actually tested red clover in people, they saw no real change in how the body handled a CYP1A2 test substance.

On top of that, Vosevi (sofosbuvir, velpatasvir, voxilaprevir) doesn't really lean on CYP1A2 to be cleared, so this particular pathway isn't a meaningful route for your hepatitis C medicine. I wouldn't expect red clover to change how well your treatment works or to add side effects through this. Still, because finishing your full course matters, it's worth running any supplement past your pharmacist or doctor first.

Here's the gist for anyone on Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) wondering about red clover. Part of this combination, especially velpatasvir and voxilaprevir, passes through a liver enzyme called CYP3A4. In a test tube, red clover's isoflavones looked like they might slow that enzyme down, which in theory could let a bit more of the medicine build up. More buildup could mean a slightly higher chance of things like headache, nausea, tiredness, or diarrhea.

But the real-world evidence is reassuring. A small human study using a standardized red clover dose found no meaningful change in a CYP3A4 test drug, so this concern is mostly theoretical. Since finishing your full hepatitis C course matters a lot, I'd still mention any supplement to your pharmacist or doctor before adding it.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
