Rabeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

Here's the simple version. Your body uses a liver enzyme called CYP2C19 to help clear rabeprazole from your system. Schisandra appears to slow that same enzyme down, at least in lab dishes and animals. If it does the same in people, rabeprazole could hang around a bit longer and its effect could be a touch stronger. With this drug that's usually mild, things like headache, nausea, or some stomach upset.

The good news: this interaction is only theoretical right now and hasn't actually been seen in humans, so I wouldn't lose sleep over it. Rabeprazole also isn't a narrow-window drug, so small changes rarely cause trouble. Still, if you plan to use schisandra regularly, give your pharmacist or doctor a quick heads-up first.

Here's the idea behind this pairing. Schisandra may speed up a liver enzyme called CYP2C9, and rabeprazole is partly handled by that same enzyme family. If schisandra revs it up, your body could clear rabeprazole a little faster, which in theory might make it work less well. That could mean your heartburn or reflux symptoms creep back, or your stomach acid isn't controlled as well as before.

The important part: this is only based on lab and animal studies, and it has never actually been shown in people. So I wouldn't lose sleep over it. Still, if you're taking rabeprazole for GERD or an ulcer and want to add schisandra, run it by your pharmacist or doctor first, and watch for any return of your symptoms.

Rabeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Schisandra tends to slow that enzyme down, so in theory rabeprazole could hang around a bit longer than usual and its effects could be slightly stronger. With a stomach acid reducer like this, that's usually not dangerous, but you might notice more of its mild side effects, such as headache, nausea, or loose stools.

I want to be honest about the strength of this concern. The evidence here comes mostly from lab and animal studies, not from people taking rabeprazole, so this is more of a theoretical heads-up than a proven problem. Food amounts of schisandra berry are unlikely to matter. If you're using concentrated supplement capsules, just run it past your pharmacist or doctor first.

About Rabeprazole

What Is Rabeprazole Used For?

Brand name: Pariet

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.

In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
