Why this applies to Vincristine

Vincristine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the basic idea: your body relies on a liver enzyme called CYP3A4 to clear vincristine out of your system. In a test tube, uva ursi seemed to slow that enzyme down. If that were to happen in real life, vincristine could linger longer and build up, which might mean stronger effects and more side effects than expected.

With vincristine that matters, because its tougher side effects include nerve problems (tingling, numbness, weakness, constipation) and low blood counts. So more drug in the body is not a good thing. That said, this is only a theoretical, lab-based concern that hasn't been seen in people. Still, since vincristine is serious chemo, please run any uva ursi use past your oncologist or pharmacist first.

Vincristine also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the basic idea: your body uses a kind of cellular pump called P-glycoprotein to move certain medicines, including vincristine, out of cells. In lab dish studies, uva ursi seemed to slow that pump down a bit. If that same thing happened in people, vincristine could linger at higher levels than expected, which in theory might mean more of its known side effects, like nerve problems (tingling, numbness, weakness), constipation, or low blood counts.

I want to be honest, though: this has only been seen in test-tube work, never in actual patients, so we really don't know if it matters. Since vincristine is serious chemotherapy, please don't start uva ursi without first checking with your oncology team or pharmacist.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Vincristine Injection Used For?

Brand name: Vincasar

Vincristine Sulfate Injection is approved to treat acute leukemia. It is also used as part of combination chemotherapy regimens for Hodgkin's disease, non-Hodgkin's malignant lymphomas, rhabdomyosarcoma (a soft-tissue cancer), neuroblastoma (a cancer of nerve tissue), and Wilms' tumor (a kidney cancer in children).

Because it works best alongside other cancer medicines, vincristine is almost always given as part of a multi-drug treatment plan rather than on its own.
