Levomilnacipran falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yerba Mate interacts with.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the gist for Fetzima (levomilnacipran). Your body uses a liver enzyme called CYP3A4 to help break this medication down. Lab studies suggest yerba mate may slow that enzyme a bit, which in theory could let levomilnacipran build up slightly. Since this drug is active on its own (not a prodrug), more in your system could mean a touch more of its usual effects, things like a faster heart rate, higher blood pressure, sweating, nausea, or trouble sleeping.

That said, I want to keep this in perspective. This concern comes only from test-tube research, not real people, and it's rated a minor interaction. A cup or two of mate likely won't be a problem for most folks. If you drink it heavily or use concentrated extracts, just mention it to your pharmacist or doctor so they can keep an eye on you.
