Yerba mate contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, it might increase the risk of bleeding when used concomitantly with these agents; however, this interaction has not been reported in humans.

Here's the gist: vorapaxar (Zontivity) is already a powerful blood thinner that keeps your platelets from clumping together, and you're usually taking it alongside aspirin and clopidogrel too. The caffeine in yerba mate has some mild platelet-slowing activity of its own in lab studies, so the worry is that piling it on top could nudge your bleeding risk a little higher.

I want to be honest about how soft this evidence is. It comes only from test-tube and animal work, and it has never actually been reported in people. A cup or two of mate is unlikely to be a problem. Still, with a drug this strong, watch for easy bruising, nosebleeds, or bleeding that won't stop, and run your mate habit by your pharmacist or doctor.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the gist for vorapaxar (Zontivity): your liver uses an enzyme called CYP3A4 to clear it from your body. Lab studies suggest yerba mate may slow that enzyme down a bit. In theory, that could let a little more vorapaxar build up, and since this is a blood thinner, the main thing you'd watch for is extra bruising or bleeding, like nosebleeds, bleeding gums, or blood in the urine or stool.

I want to keep this in perspective, though. This is rated a minor interaction and it comes only from test-tube research, not from studies in real people. A cup or two of mate is unlikely to cause trouble. Still, since vorapaxar is usually taken with aspirin and Plavix, it's worth mentioning your mate habit to your pharmacist or doctor, especially if you notice any unusual bleeding.

What Is Vorapaxar Used For?

Brand name: Zontivity

Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.

Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
