These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
1,3-dmba interacts with whole categories of medication. Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls into 2 of those categories, so it can interact with 1,3-dmba in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
STIMULANT DRUGS
1 of 2Why this applies to Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine
Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under STIMULANT DRUGS — the group 1,3-dmba interacts with.
Animal research shows that 1,3-DMBA has stimulant effects, such as increasing blood pressure and heart rate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination cold-and-pain product, sometimes sold as Hycomine Compound, already carries two stimulant-type ingredients: caffeine and phenylephrine (a decongestant that tightens blood vessels). Stacking 1,3-DMBA on top of those adds yet another stimulant pushing in the same direction. The worry here is mainly about your heart and blood vessels. Together they could nudge your blood pressure and heart rate higher, and you might feel jittery, anxious, or notice a pounding or racing heartbeat.
To be fair, this concern is theoretical and comes mostly from animal data, so it is not firmly proven in people. Still, 1,3-DMBA itself is a risky, FDA-flagged ingredient I would steer clear of. Please run this past your pharmacist or doctor before mixing them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 2Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
In vitro research shows that 1,3-DMBA weakly inhibits CYP2D6 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Let me walk through this combo. Lab-dish studies suggest 1,3-DMBA weakly blocks an enzyme called CYP2D6, and the hydrocodone in this cold-and-pain product is partly handled by that same enzyme. Here's the twist: hydrocodone is a prodrug that CYP2D6 turns into a stronger painkiller (hydromorphone). So if that enzyme is slowed, you may get slightly less pain relief, not more. The chlorpheniramine in this product also leans on CYP2D6, so its drowsiness could nudge up a bit.
Honestly, the bigger worry is the stimulant itself. 1,3-DMBA can spike heart rate and blood pressure, and so can the caffeine and phenylephrine already in this medicine. That stacking is the real concern. This enzyme effect is only theoretical, so don't panic, but please run it by your pharmacist.
