Why this applies to Ciclesonide

Ciclesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the good news on this pairing. Ciclesonide is an inhaled steroid that your body activates and then clears using the CYP3A4 enzyme. Plumbagin, a compound in Ceylon Leadwort, has slowed this same enzyme in lab dish studies. In theory, that could let a bit more ciclesonide hang around, nudging up the small risk of steroid-type effects like mouth or throat irritation, hoarseness, or oral thrush.

But I want to keep this in perspective. This interaction has only been seen in test tubes, never reported in real people, so it's largely theoretical. Since you breathe in only tiny amounts of ciclesonide, any real-world effect is likely minor. Still, it's worth a quick chat with your pharmacist or doctor before combining them.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the thing to keep in mind with this pairing. Ciclesonide (Alvesco, Omnaris, Zetonna) is an inhaled or nasal corticosteroid, so part of how it helps you is by calming down inflammation and dialing back the immune response in your airways. Ceylon Leadwort contains a compound called plumbagin that, in lab and animal studies only, has shown its own immune-dampening activity. The worry on file is purely theoretical: stacking two things that affect the immune system might add up in ways we can't fully predict.

I want to be honest that the evidence here is weak, and inhaled ciclesonide mostly works right at the airways with very little reaching the rest of your body. Still, since we just don't have good human data on Ceylon Leadwort, please run it by your pharmacist or doctor before combining them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Ciclesonide Oral Inhalation Used For?

Brand names: Omnaris, Alvesco, Zetonna

Alvesco (ciclesonide) inhaler is approved for the maintenance treatment of asthma as a preventive therapy in adults and children 12 years of age and older. It is taken regularly to reduce airway inflammation and help prevent asthma symptoms — not to relieve a sudden attack.

Alvesco is not a rescue inhaler and should not be used to treat sudden wheezing or acute asthma episodes. It is also not approved for children under 12.
