Abiraterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist for anyone on abiraterone (Zytiga): your liver uses an enzyme called CYP3A4 to help clear this medicine. In lab dish studies, chanca piedra appears to slow that same enzyme down. In theory, that could let abiraterone build up a bit higher than expected, which might mean more of its usual effects, things like liver enzyme changes, higher blood pressure, fluid swelling, or low potassium.

I want to be honest about the strength of this, though. It's based only on test-tube research and has never actually been reported in people taking this combo, so the real-world risk may be small. Since this is serious cancer therapy, don't change anything on your own. Just give your oncologist or pharmacist a heads-up before adding chanca piedra so they can weigh in.

About Abiraterone

What Is Abiraterone Used For?

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
