Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear acalabrutinib (Calquence) out of your system. In lab studies, chanca piedra appears to slow that same enzyme down. If that effect holds true in people, it could let acalabrutinib build up higher than intended, which may mean more of its known side effects, things like bruising or bleeding more easily, headaches, low blood counts, fatigue, or an irregular heartbeat.

I want to be honest about the strength of this, though. So far this is only seen in test-tube research and has not been reported in actual patients, so it's more of a theoretical caution than a proven problem. Because acalabrutinib is a serious cancer medication, please don't start or stop the herb on your own. Run it by your oncologist or pharmacist first.

What Is Acalabrutinib Used For?

Brand name: Calquence

Acalabrutinib treats certain blood cancers in adults. These are mantle cell lymphoma (MCL), chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL).

For MCL, it is used with bendamustine and rituximab in people not yet treated who cannot have an autologous stem cell transplant. It is also used alone in people with MCL who have had at least one earlier treatment. Calquence capsules and Calquence tablets carry these same uses.
