Acetaminophen, Codeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the honest picture with your Tylenol with Codeine. The interaction on file is based only on lab (test tube) studies suggesting chanca piedra may speed up an enzyme called CYP1A2. That particular enzyme isn't the main one that handles codeine (codeine is activated mostly by CYP2D6), so the practical effect here is uncertain and very theoretical. It has never actually been reported in people.

So I wouldn't lose sleep over this one. If anything, faster metabolism could slightly weaken effects rather than cause harm. Still, because this combo affects your pain control and breathing, it's worth a quick chat with your pharmacist or doctor before pairing them, especially if you take chanca piedra in concentrated supplement doses.
