Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Your body relies on a liver enzyme called CYP3A4 to help clear alpelisib (Piqray or Vijoice) from your system. In lab tests, chanca piedra appears to slow this same enzyme down. If that happened in a person, alpelisib could build up to higher levels than expected, which might mean stronger side effects like high blood sugar, diarrhea, rash, nausea, or mouth sores.

Here's the reassuring part: this is only a theoretical concern based on test-tube research, and it has never been reported in actual patients. Still, alpelisib is a serious cancer medication, so I wouldn't add chanca piedra on your own. Please check with your oncologist or pharmacist first so they can weigh in.

About Alpelisib

What Is Alpelisib Used For?

Brand name: Piqray

Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.

Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
