Antithrombin III falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Antithrombin III (Thrombate III) is a blood thinner given through your vein, usually in a hospital or clinic setting, to lower your risk of dangerous clots. The concern with chanca piedra is simple: both can nudge your blood toward bleeding more easily. One compound found in the herb seemed to slow platelets from clumping together in a lab dish, and on top of an anticoagulant that could, in theory, add up to a higher bleeding risk.

I want to be honest about the strength of this, though. This is theoretical only, based on test-tube work, and it has never actually been reported in people. Still, since Antithrombin III is reserved for serious clotting situations, it is worth mentioning the herb to your doctor or pharmacist so they can keep an eye out for things like easy bruising or unusual bleeding.
