Buproprion, Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Auvelity combines bupropion and dextromethorphan. Here the bupropion piece actually works to slow down how fast dextromethorphan gets broken down, which is on purpose in this combo. Layered on top of that, lab studies suggest chanca piedra may gently block a liver enzyme called CYP3A4, which helps clear both ingredients. In theory, that could nudge their levels a little higher.

If levels did rise, you might notice more of the usual effects, things like dizziness, dry mouth, jitteriness, trouble sleeping, or feeling foggy. I want to be honest though: this is only seen in a test tube, never reported in real people, so the real-world risk looks small. Still, it is worth a quick mention to your pharmacist or doctor before adding the herb.
