Cilostazol falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the basic idea with cilostazol (Pletal): its whole job is to keep your platelets from clumping together so blood flows more easily to your legs. Chanca piedra may have a mild blood-thinning quality of its own. In a lab dish, one of its compounds slowed platelet activity, so the concern is that stacking the two could nudge you toward easier bruising or bleeding, things like nosebleeds, bleeding gums, or cuts that take longer to stop.

I want to be honest about how soft this evidence is. It's theoretical and comes only from test-tube work, never confirmed in real people, so for most folks the practical risk is probably small. Still, since both can affect bleeding, please check with your pharmacist or doctor before combining them, especially if you have any other bleeding risk.

Cilostazol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the thing to keep in mind: your body relies on a liver enzyme called CYP3A4 to help clear cilostazol (Pletal) out of your system. In lab tests, chanca piedra appears to slow that same enzyme down. If that held true in people, less of the drug would be broken down, so its levels could creep up and its effects could feel stronger than expected.

For cilostazol, stronger effects might mean more headache, dizziness, a pounding or fast heartbeat, diarrhea, or easier bruising or bleeding. I want to be honest, though: this is only a theoretical concern from test-tube research, and it hasn't been seen in real patients. Still, since cilostazol affects platelets, it's worth a quick chat with your pharmacist or doctor before combining the two.
