Codeine, Paracetamol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the honest picture with co-codamol (codeine plus paracetamol). The concern on file is that chanca piedra might speed up a liver enzyme called CYP1A2. But this enzyme isn't the main one that handles codeine or paracetamol, so the practical effect on your pain relief is likely small. Codeine is actually a prodrug that relies mostly on a different enzyme (CYP2D6) to turn into its active form, which CYP1A2 doesn't really control.

On top of that, this is only a lab-dish finding, never seen in real people, so I wouldn't lose sleep over it. Still, it's worth a quick word with your pharmacist or doctor before combining them, especially since paracetamol is processed by the liver and chanca piedra is often used for liver health.
