Enasidenib falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist for Idhifa (enasidenib), your leukemia medicine. Lab studies (in test tubes, not people) suggest chanca piedra can rev up a liver enzyme called CYP1A2. Enasidenib is partly handled by that enzyme, so in theory, speeding it up could clear the drug from your body a little faster and leave less of it working against the cancer.

I want to be honest with you though: this is purely theoretical and has never actually been seen in real patients. The concern is reduced benefit rather than new side effects. Still, because this drug treats a serious cancer, I wouldn't risk even a small dip. Please run any chanca piedra use past your oncologist or pharmacist before starting it.

Here's the gist for your leukemia medicine, enasidenib (Idhifa). Your body uses a liver enzyme called CYP3A4 to help clear this drug. Lab studies suggest chanca piedra can slow that same enzyme down. If that happened in a real person, enasidenib could build up a bit more than expected, which might mean stronger effects and a higher chance of side effects like nausea, low appetite, fatigue, or changes in your blood counts and bilirubin.

The important part: this is only a theoretical, test-tube finding, and it has never actually been reported in people. Still, since enasidenib is a serious cancer drug, I'd play it safe. Please check with your oncologist or pharmacist before adding chanca piedra rather than starting or stopping anything on your own.

What Is Enasidenib Used For?

Brand name: Idhifa

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
