Theoretically, concomitant use of chanca piedra with antihypertensive drugs might have additive blood pressure lowering effects.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the quick picture. Esmolol is a fast-acting beta blocker given by IV, usually in a hospital or monitored setting to slow a racing heart and bring down blood pressure quickly. Chanca piedra has shown a mild blood-pressure-lowering effect in some lab and animal studies, so in theory, pairing the two could nudge your blood pressure or heart rate a little lower than expected.

In real life this is a very minor, mostly theoretical concern. Esmolol is carefully dosed and watched by the medical team, and one human study found chanca piedra didn't actually lower blood pressure in most patients. Still, since esmolol is used in situations where blood pressure matters, it's worth mentioning any supplement to your care team so they have the full picture.
