Chanca Piedra and Ethinyl estradiol, Levonogestrel

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Chanca Piedra interacts with whole categories of medication. Ethinyl estradiol, Levonogestrel falls into 2 of those categories, so it can interact with Chanca Piedra in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Ethinyl estradiol, Levonogestrel

Ethinyl estradiol, Levonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the simple version for your birth control pill (the Lybrel combination of ethinyl estradiol and levonorgestrel). In lab studies, chanca piedra seems to speed up a liver enzyme called CYP1A2, which helps process estrogen. In theory, that faster clearance could lower the amount of hormone in your blood. Since these pills work best when hormone levels stay steady, a drop could, on paper, make the pill less reliable and possibly lead to breakthrough bleeding or even reduced protection against pregnancy.

Important: this is only a test-tube finding and has never actually been seen in people, so I don't want to scare you. Still, because reliable birth control matters so much, please check with your pharmacist or doctor before combining the two rather than stopping anything on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Ethinyl estradiol, Levonogestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your birth control pill, Lybrel, contains two hormones (ethinyl estradiol and levonorgestrel) that your body partly clears using a liver enzyme called CYP3A4. In lab dish studies, chanca piedra appears to slow down that same enzyme. In theory, that could let a bit more hormone hang around in your system, which might mean a slightly higher chance of side effects like nausea, breast tenderness, headaches, or spotting.

I want to be honest though: this has only been seen in test tubes, never in real people, so it's a fairly weak, theoretical concern. Neither of these hormones is a prodrug, so the direction makes sense (more drug, not less). Before adding chanca piedra, check with your pharmacist or doctor so they can keep an eye on you.

About Chanca Piedra

What is Chanca Piedra?

Chanca Piedra
  • Kidney stones
  • Gallstones
  • Liver support
  • Urinary tract health
  • Digestive complaints

Chanca piedra is a tropical herb traditionally used as a 'stone breaker' for kidney and gallstones, and for liver and urinary health. Human evidence for these uses is limited and mostly small or preliminary, so it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a medical condition or take medications.
