Why this applies to Etonogestrel

Etonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist for your Implanon implant. In lab studies, chanca piedra appears to speed up an enzyme called CYP1A2, which helps break down certain medicines. Etonogestrel is the active hormone in your implant (not a prodrug), so if this herb sped up its breakdown, the worry would be slightly lower hormone levels. In theory, that could mean weaker pregnancy protection and possibly some breakthrough bleeding or spotting.

Now, please don't panic. This is only a test-tube finding, and it has never been reported in real people. Etonogestrel also isn't mainly cleared by CYP1A2, so the real-world risk looks small. Still, since this implant is your birth control, it's worth a quick chat with your pharmacist or doctor before adding chanca piedra.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

The etonogestrel in your Implanon implant is cleared from your body by a liver enzyme called CYP3A4. In lab dish studies, chanca piedra appears to slow that enzyme down. If that same effect happened in real life, your body would break down the hormone a little more slowly, which could let levels drift up a bit. Etonogestrel is not a prodrug, so more of it in your system simply means more hormone, which might mean side effects like irregular spotting, breast tenderness, headaches, or mood changes.

Here's the reassuring part: this is only a theoretical concern from test-tube work, and it has never been reported in actual people. I wouldn't lose sleep over it, but please mention the supplement to your pharmacist or doctor so they can keep an eye on things.
