Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist with fosaprepitant (Emend Injection), which is given through an IV to help prevent nausea and vomiting from chemotherapy. Once in your body it converts to aprepitant, and that active form is partly cleared by a liver enzyme called CYP3A4. In lab studies, chanca piedra appears to slow that same enzyme down. In theory, that could let aprepitant linger a bit longer and feel stronger, possibly nudging up side effects like tiredness, hiccups, headache, or constipation.

I want to be straight with you though: this is based only on test-tube data and hasn't actually been seen in real people, so the concern is more cautious than proven. Since fosaprepitant is usually given short-term during chemo, just let your oncology team and pharmacist know you're using chanca piedra so they can keep an eye on things.
