Lepirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Lepirudin (Refludan) is a powerful injectable blood thinner, a direct thrombin inhibitor usually given in the hospital to people who can't safely use heparin. It works right away on the clotting system, so anything else that nudges your blood toward less clotting deserves attention.

Here's the concern with chanca piedra: in lab studies, one of its compounds seemed to keep platelets from sticking together. That points to a possible added bleeding effect on top of lepirudin, things like easy bruising, nosebleeds, or bleeding that's slow to stop. But honestly, this is theoretical only and has never been shown in real people. Since lepirudin is given under close medical supervision, just make sure your care team knows about every supplement you take so they can monitor you.
