Lurbinectedin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the situation with these two. Your body relies on a liver enzyme called CYP3A4 to break down and clear lurbinectedin. In lab studies (test tube only, not in actual people), chanca piedra appears to slow that same enzyme down. If that held true in the body, lurbinectedin could be cleared more slowly and build up to higher levels than intended.

Because lurbinectedin is a potent chemotherapy drug, higher levels could mean worse side effects, things like a drop in blood counts (raising infection and bleeding risk), severe fatigue, and nausea. To be clear, this is theoretical and hasn't been seen in real patients. Still, with cancer treatment I'd play it safe. Please run any supplement by your oncologist or pharmacist first.

About Lurbinectedin Injection

What Is Lurbinectedin Injection Used For?

Brand name: Zepzelca

Zepzelca (lurbinectedin) injection is approved for adults with small cell lung cancer (SCLC) in two settings. First, Zepzelca is used in combination with atezolizumab (or atezolizumab and hyaluronidase-tqjs) as maintenance treatment for extensive-stage small cell lung cancer (ES-SCLC) in patients whose disease did not progress after initial treatment with those same immunotherapy drugs plus carboplatin and etoposide.

Second, Zepzelca is approved as a single agent for adults with metastatic small cell lung cancer whose cancer has worsened during or after platinum-based chemotherapy. This second use was granted accelerated approval based on tumor response rates, and continued approval depends on results from confirmatory clinical trials.
