pioglitazone, metformin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Let me put this in plain terms. ActoPlus Met combines two diabetes medicines: pioglitazone and metformin. Pioglitazone is partly cleared by a liver enzyme called CYP3A4. In a test-tube study, chanca piedra slowed that enzyme down, so in theory it could let pioglitazone build up a bit. That might mean stronger effects, like more fluid retention, swelling, or weight gain. Metformin is handled by your kidneys, not this enzyme, so it isn't really affected here.

The big thing to know is this is theoretical only, based on lab work, and hasn't been seen in real people. So I wouldn't lose sleep over it, but do mention chanca piedra to your pharmacist or doctor before starting it.

Theoretically, concomitant use with antidiabetes drugs might affect glucose control and increase the risk of hypoglycemia.
Animal research suggests that chanca piedra can have hypoglycemic effects. However, a small clinical study in adults with diabetes shows that chanca piedra extract 25 grams orally daily for 1 week does not lower fasting or postprandial blood glucose levels.

Here's the gist with ActoPlus Met (that's pioglitazone plus metformin, both used to lower blood sugar in type 2 diabetes). On paper, chanca piedra has shown some blood-sugar-lowering activity in animal studies, so the worry is that stacking it with diabetes medicine could push your sugar a little too low. Low blood sugar can show up as shakiness, sweating, dizziness, or feeling foggy.

That said, the real-world evidence here is thin. A small human study found that chanca piedra didn't actually lower blood sugar, so this interaction is considered minor and unlikely. I wouldn't lose sleep over it, but it's smart to keep an eye on your readings if you try it, and check with your pharmacist or doctor first.
