Tolterodine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the simple version: your body uses an enzyme called CYP3A4 to help clear tolterodine (Detrol) out of your system. Lab studies suggest chanca piedra can slow that enzyme down. If that happened in real life, tolterodine could build up a bit more than usual, and that may nudge up the typical side effects of this bladder medicine, things like dry mouth, constipation, blurry vision, drowsiness, or a faster heartbeat.

The honest caveat: this is only seen in test-tube research, not in actual people, so it's theoretical and may not matter much in practice. Still, it's worth a quick chat with your pharmacist or doctor before combining them, rather than starting or stopping anything on your own.

Tolterodine — available as tablets (including Detrol and Tolterodine Tartrate tablets) and extended-release capsules (including Detrol LA and Tolterodine Tartrate extended-release capsules) — is prescribed for overactive bladder. It targets three main symptoms: a sudden, hard-to-control urge to urinate (urgency), urinating too often (frequency), and leaking urine when that urge hits (urge urinary incontinence).
