Treprostinil (IV) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the thing about treprostinil (Remodulin): besides widening blood vessels in the lungs, it also makes platelets a little less sticky, so it can mildly raise bleeding risk on its own. Chanca piedra may do something similar. In lab studies, one of its compounds (methyl brevifolincarboxylate) slowed platelet clumping, though this hasn't actually been seen in people.

So the worry is a theoretical doubling-up effect: more easy bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding from cuts. The evidence here is weak and based on test-tube work, not human studies, so I wouldn't lose sleep over it. Still, since you're on an IV medication for a serious heart-lung condition, please check with your doctor or pharmacist before adding chanca piedra.

What Is Treprostinil Injection Used For?

Brand name: Remodulin

Treprostinil injection is used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — high blood pressure in the arteries of the lungs — to reduce symptoms during exercise. It has been studied in patients with moderate to severe PAH symptoms (NYHA Class II–IV) caused by idiopathic or heritable PAH, PAH linked to congenital heart defects, or PAH associated with connective tissue diseases.

It is also used for patients with PAH who need to transition off epoprostenol (another PAH medicine), to slow the rate of disease worsening during that switch. The benefits and risks of transitioning should be carefully weighed by the prescriber.
