Cola Nut and Aspirin, Chlorpheniramine, Phenylpropanolamine

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Cola Nut interacts with whole categories of medication. Aspirin, Chlorpheniramine, Phenylpropanolamine falls into 3 of those categories, so it can interact with Cola Nut in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 3

Why this applies to Aspirin, Chlorpheniramine, Phenylpropanolamine

Aspirin, Chlorpheniramine, Phenylpropanolamine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Cola Nut interacts with.

Theoretically, cola nut may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Cola nut contains caffeine. Caffeine is reported to have antiplatelet activity. This interaction has not been reported in humans.
Severity Moderate Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Here's the short version: this combo product already contains aspirin, which thins your blood a bit by making platelets less sticky. Cola nut is mostly caffeine, and in lab studies caffeine has shown some mild platelet-slowing activity too. So in theory, stacking the two could add up and slightly raise your chance of bleeding or bruising.

That said, I want to keep this in perspective. This concern comes only from test-tube and animal work, and it has not actually been reported in people. A normal cup of cola-flavored or caffeinated drink is very unlikely to matter. Concentrated cola nut supplements are the bigger question mark. If you bruise easily, notice bleeding gums, or take aspirin daily, just run it past your pharmacist or doctor first.

STIMULANT DRUGS

2 of 3

Aspirin, Chlorpheniramine, Phenylpropanolamine also falls under STIMULANT DRUGS.

Theoretically, concomitant use might increase stimulant adverse effects.
Cola nut contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here is the piece to pay attention to in this combo product: phenylpropanolamine. That is a decongestant-type stimulant that can nudge your heart rate and blood pressure up. Cola nut is basically a caffeine source, and caffeine is a stimulant too. Stack the two together and you can pile on the same effects, so you might feel jittery, anxious, or shaky, notice your heart racing or pounding, have trouble sleeping, or see your blood pressure climb.

To be fair, this is mostly a theoretical concern based on expert opinion rather than hard studies, and a cola flavored drink is unlikely to be a problem. Concentrated cola nut supplements are where it adds up. If you have heart issues or high blood pressure, check with your pharmacist or doctor first.

PHENYLPROPANOLAMINE

3 of 3

Aspirin, Chlorpheniramine, Phenylpropanolamine also falls under PHENYLPROPANOLAMINE.

Theoretically, phenylpropanolamine might increase the risk of hypertension, as well as the levels and adverse effects of the caffeine in cola nut.
Cola nut contains caffeine. Concomitant use of phenylpropanolamine and caffeine might cause an additive increase in blood pressure. Phenylpropanolamine also seems to increase caffeine serum levels.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the thing to know about this combo. Your Alka-Seltzer Plus Cold contains phenylpropanolamine, a decongestant that can nudge up your blood pressure on its own. Cola nut is basically a concentrated source of caffeine, which is also a stimulant. Put them together and the blood pressure bump can stack up. On top of that, phenylpropanolamine seems to slow how your body clears caffeine, so the caffeine hangs around longer and hits harder.

In real life that can mean a racing or pounding heart, jitteriness, headache, trouble sleeping, and higher blood pressure readings. None of these drugs is a prodrug here, so the direction is simply more stimulant effect. If you have heart issues or high blood pressure, please check with your pharmacist or doctor before pairing them.

Cola nut is a caffeine-containing seed from West Africa used mainly as a natural stimulant for energy and alertness.
