Paliperidone Palmitate falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with this pairing. Paliperidone (Invega Trinza) is a long-acting injection that can stretch out the heart's electrical rhythm a little, something we call QT prolongation. Ephedra is a strong stimulant that can do the same thing, plus it speeds up the heart and raises blood pressure. Stack the two together and you may add up those effects, which can raise the risk of a dangerous heart rhythm, lightheadedness, fainting, or a racing or pounding heartbeat.

This worry is partly theoretical and based on limited evidence, but the stakes are high, and ephedra is already banned in U.S. supplements for safety reasons. With a long-acting shot like this one, the drug stays in your system for months, so you can't simply skip a dose. Please talk with your doctor or pharmacist about safer options before using any ephedra product.

What Is Paliperidone Injection Used For?

Brand name: Invega Trinza

Paliperidone injection is used to treat schizophrenia and schizoaffective disorder in adults. Different branded products are approved for specific treatment situations.

Invega Sustenna (once-monthly injection) treats both schizophrenia and schizoaffective disorder. Invega Trinza (every-three-month injection) and Invega Hafyera (every-six-month injection) treat schizophrenia in adults who have already been stabilized on a paliperidone palmitate injectable. Erzofri extended-release is a once-monthly injectable for schizophrenia and schizoaffective disorder in adults.
