Pentazocine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the basic idea with ephedra and pentazocine (Talwin). Your liver uses an enzyme called CYP1A2 to help break down pentazocine, and some early animal research hints that ephedra may speed that enzyme up. If that happened in people, your body could clear the pentazocine a little faster, which might mean less pain relief than you expect.

I want to be honest about the strength of this: it's based on lab and animal data only, not human studies, so it's more theoretical than proven. The bigger concern is ephedra itself, which is banned in U.S. supplements because it can raise blood pressure and strain the heart. Please talk with your pharmacist or doctor before combining anything like this.
