Pseudoephedrine, Terfenadine falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

This combination has a double concern. Your medicine already pairs pseudoephedrine, a stimulant, with terfenadine, an older antihistamine that's well known for affecting the heart's electrical rhythm (the QT interval). Adding ephedra piles on more stimulant load and its own QT effects, and the two can add together. The worry isn't a stronger cold remedy, it's the heart: a faster pulse, palpitations, higher blood pressure, and in the worst case a dangerous irregular rhythm.

It's worth knowing terfenadine was pulled from the U.S. market years ago for exactly these rhythm risks, and ephedra is banned in supplements here too. The evidence here is moderate and partly theoretical, but the stakes are serious. Please don't combine these; talk with your pharmacist or doctor about safer options.

Here's the tricky part with this combo product: it already contains pseudoephedrine, which is a close chemical cousin of the stimulants in ephedra (ephedrine and pseudoephedrine are nearly the same molecule). So stacking ephedra on top of a medicine that's already supplying pseudoephedrine is essentially doubling up on the same kind of stimulant. The worry is additive: a faster or pounding heartbeat, jitteriness, trouble sleeping, and most importantly a meaningful jump in blood pressure that could raise the risk of dangerous heart and stroke events.

This is flagged as a major concern, though the evidence here is mostly theoretical and anecdotal rather than proven in studies. Even so, given that ephedra is banned in U.S. supplements for safety reasons, I wouldn't combine them. Please check with your pharmacist or doctor before taking these together.
