Hordenine and Mirtazapine

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Mirtazapine

Mirtazapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Hordenine interacts with.

Hordenine weakly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, hordenine might increase the levels of CYP2D6 substrates.
Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for mirtazapine (Remeron): your liver uses an enzyme called CYP2D6 as one of the ways it clears this drug. In lab (test tube) studies, hordenine appears to weakly block that enzyme. In theory, that could let a bit more mirtazapine hang around in your body, which might nudge up its usual effects like drowsiness, dizziness, dry mouth, increased appetite, or feeling groggy the next morning.

I want to be honest about how soft this is. The evidence is only laboratory-based, the inhibition is described as weak, and the concern is rated minor. Mirtazapine also relies on several enzymes, not just this one, so a real-world problem is unlikely for most people. Still, if you notice extra sleepiness or sluggishness, check in with your pharmacist or doctor.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Hordenine

What is Hordenine?

Hordenine
  • Energy and stimulation
  • Athletic and workout performance
  • Weight loss/fat burning
  • Focus and mood
  • Appetite control

Hordenine is a natural alkaloid found in barley and some cacti that is marketed as a stimulant for energy, focus, and fat loss, but solid human evidence for these benefits is lacking. Its safety in people has not been well studied, so caution is warranted, and it should be avoided in pregnancy, breastfeeding, and by people with heart conditions.

About Mirtazapine

What Is Mirtazapine Used For?

Brand name: Remeron

Mirtazapine tablets, mirtazapine orally disintegrating tablets, Remeron, and RemeronsолTab are all indicated for the treatment of major depressive disorder (MDD) in adults. Major depressive disorder involves a persistently low or sad mood most days, often accompanied by loss of interest, sleep changes, fatigue, and difficulty concentrating.

The effectiveness of mirtazapine tablets in maintaining a response for up to 40 weeks after an initial treatment period has also been demonstrated. Patients should be reassessed periodically to determine whether continued treatment is needed.
