Why this applies to Carvedilol

Carvedilol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist with carvedilol and lentinan: your liver uses an enzyme called CYP2D6 to break carvedilol down. Lab and animal research hints that lentinan might rev up that enzyme, which could clear carvedilol from your body a little faster and weaken its effect. Carvedilol isn't a prodrug, so faster clearance means less drug working, not more.

In real life, that could mean your blood pressure or heart rate isn't as well controlled as expected. The honest part: this has never actually been seen in people, only in test-tube and animal studies, so it's more theoretical than proven. Still, if you take carvedilol for heart failure or blood pressure, check with your pharmacist or doctor before adding lentinan, and keep an eye on your readings.

What Is Carvedilol Used For?

Carvedilol is used to treat three main conditions: mild-to-severe chronic heart failure, high blood pressure (hypertension), and reduced heart pumping function (left ventricular dysfunction) in people who have survived a heart attack. For heart failure, it is usually prescribed alongside other medications such as diuretics, ACE inhibitors, and digitalis to help people live longer and reduce the chance of hospital stays.

For people who have had a heart attack and whose heart is not pumping as well as it should, carvedilol is used to lower the risk of dying from heart-related causes. For high blood pressure, it can be used on its own or combined with other blood pressure medicines.
