This is one of those interactions that looks scary on paper but very likely won't matter in real life. The caffeine portion of this combo pill is handled by a liver enzyme called CYP1A2. In lab dishes, red clover slows that enzyme down a little, which in theory could let caffeine linger and bump up jitteriness, a racing heartbeat, or trouble sleeping. But here's the good news: a real human study using standardized red clover isoflavones showed no actual change in how the body cleared caffeine.
The acetaminophen and codeine parts aren't the concern here. So I'd call this a minor, mostly theoretical issue. Still, if you notice extra restlessness or a faster pulse, mention it to your pharmacist before changing anything.

However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist for this combo pill (which mixes acetaminophen for pain, caffeine, and codeine). In the lab, red clover's plant compounds looked like they might slow down CYP3A4, an enzyme that helps process many medicines. If that happened, drug levels could creep up a little. But when researchers actually tested red clover in people, it did not meaningfully change how a CYP3A4 medicine was handled. So the real-world concern here is small.
One thing worth knowing: codeine is a prodrug, meaning your body converts it into morphine to relieve pain, and that step relies mostly on a different enzyme (CYP2D6), not CYP3A4. So red clover is unlikely to meaningfully change your pain relief or side effects like drowsiness or constipation. Still, it's smart to run any new supplement by your pharmacist or doctor first.

Red clover contains genistein. Taking genistein 1 gram daily for 14 days seems to inhibit caffeine clearance and metabolism in healthy females. However, this effect does not seem to occur with the lower amounts of genistein found in red clover. A clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine.

In high lab doses, a red clover isoflavone called genistein can slow how fast your body clears caffeine.
