Alogliptin, Pioglitazone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version for Oseni (which combines alogliptin and pioglitazone): on paper, red clover could slow down the CYP3A4 enzyme that helps clear pioglitazone, which might let a bit more build up. In real life, though, this worry hasn't panned out. A study using red clover standardized to 60 mg isoflavones twice daily found it didn't change blood levels of a CYP3A4 test drug at all. So any effect on your Oseni is rated minor and unlikely.

If pioglitazone levels did creep up, you might in theory notice more swelling, weight gain, or low blood sugar. That's a very low risk here, but it never hurts to mention your supplements to your pharmacist or doctor, especially if you watch for fluid retention.
