In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for clomipramine (Anafranil): your liver uses an enzyme called CYP1A2 to help clear it. In a test tube, red clover's isoflavones can slow that enzyme down. If that happened in your body, clomipramine could build up a bit, which in theory might mean more of its usual side effects, things like dry mouth, constipation, drowsiness, dizziness, or a faster heartbeat. Clomipramine works as-is (it's not a prodrug that needs activating), so blocking this enzyme would push levels up, not down.
The good news is the real-world evidence is weak. This concern comes from lab studies only, and a small human study found red clover did not change how a CYP1A2 test drug was handled. Still, since clomipramine needs careful dosing, please check with your pharmacist or doctor before combining them.

Here's the situation with red clover and your clomipramine. Your body partly relies on a liver enzyme called CYP2C19 to clear this medication. In lab studies (test tube only), red clover's plant estrogens appear to slow that enzyme down a little. If that were to happen in real life, your clomipramine could build up slightly, which might mean more of the usual side effects like dry mouth, constipation, drowsiness, blurred vision, or a faster heartbeat.
The honest truth is this interaction has never actually been reported in people. It comes only from cell studies, and the effect looked weak. So I wouldn't lose sleep over it, but since clomipramine isn't a gentle drug, it's worth a quick mention to your pharmacist or doctor before you start red clover.
