Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the gist for dronabinol (Marinol, Syndros) and red clover. Your liver uses an enzyme called CYP2C9 to help break down dronabinol. In lab dishes, red clover's plant estrogens look like they might slow that enzyme down a little. If that happened in real life, dronabinol could linger a bit longer and you might notice more of its usual effects, things like drowsiness, dizziness, a foggy or "high" feeling, or a faster heartbeat.

But here's the reassuring part: this is only theoretical, based on test-tube work. When researchers actually tested red clover in people, it did not change how a similar CYP2C9 drug was cleared. So a real problem is unlikely. Still, since dronabinol affects how you feel, mention any supplements to your pharmacist or doctor first.

Here's the short version: your body clears dronabinol using a liver enzyme called CYP3A4, and red clover's plant compounds can, at least in a test tube, slow that same enzyme down. In theory, if that enzyme runs slower, dronabinol could hang around a bit longer and you might notice more of its usual effects, like drowsiness, dizziness, a foggy or "high" feeling, or a faster heartbeat.

The reassuring part: this is mostly a theoretical concern. When researchers actually tested standardized red clover in people using a similar CYP3A4 medication, they saw no real change. So a meaningful interaction here is unlikely. Still, since dronabinol already affects how alert you feel, it's worth a quick word with your pharmacist or doctor before combining the two.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
