Felodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist for felodipine (Plendil): your body relies on an enzyme called CYP3A4 to break this medication down. In the test tube, red clover looked like it might slow that enzyme a little, which in theory could let felodipine build up and nudge its effects stronger. With this drug, that might show up as extra dizziness, flushing, ankle swelling, or blood pressure dropping more than expected.

The reassuring part: a real-world study in people found red clover didn't actually change levels of a similar CYP3A4 drug. So this concern is mostly theoretical and unlikely. Still, since you'd be watching your blood pressure, it's worth a quick mention to your pharmacist or doctor before adding red clover.
