Imipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the gist for imipramine (Tofranil): your liver uses an enzyme called CYP1A2 as one way to clear it. In a test tube, red clover's plant estrogens can slow that enzyme down. Here's the short version: your body uses a liver enzyme called CYP2C19 as one of the ways it clears imipramine. In a test tube, red clover's plant estrogens appeared to slow that enzyme down a little. If that happened in real life, imipramine could build up slightly, which in theory might nudge up its usual side effects, things like dry mouth, constipation, drowsiness, dizziness on standing, or a faster heartbeat.

That said, I don't want to alarm you. This is theoretical and comes only from lab work, not from anything seen in actual patients, and our records rate it as minor and unlikely. Imipramine works as-is (it isn't a prodrug that needs activating), so the concern is mild over-effect, not loss of benefit. Still, since imipramine can be sensitive, mention the red clover to your pharmacist or doctor first.

That said, this is a very weak, mostly test-tube concern. A small human study actually found red clover did not change how a CYP2C9 drug was handled. So a real-world problem here is unlikely. I wouldn't lose sleep over it, but since imipramine affects mood and the heart, it's still worth a quick mention to your pharmacist or doctor before adding red clover, especially at concentrated supplement doses.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
