Why this applies to Riluzole

Riluzole falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the short version: your body relies on a liver enzyme called CYP1A2 to clear riluzole out. In test-tube studies, red clover looked like it might slow that enzyme down a little. If that actually happened in people, riluzole could build up slightly, which in theory might mean more of its usual side effects like nausea, dizziness, tiredness, or changes in liver enzymes.

The good news is that this concern is pretty weak. A real clinical study using standardized red clover isoflavones found no meaningful effect on a CYP1A2-cleared drug. So this is mostly a theoretical, lab-based worry rather than a proven problem. Riluzole is not a prodrug, so it doesn't flip directions. Still, since riluzole already needs liver monitoring, it's worth a quick mention to your pharmacist or doctor before starting red clover.

Riluzole is approved to treat amyotrophic lateral sclerosis (ALS) — the progressive neurological disease also known as Lou Gehrig's disease. This indication is shared across all available riluzole products, including tablets (Rilutek, riluzole), the dissolving oral film (Exservan), and the oral suspension (Tiglutik).

Tiglutik oral suspension is notable in that it can also be given through a PEG-tube (percutaneous endoscopic gastrostomy tube) for patients who have difficulty swallowing.
