Why this applies to Simvastatin

Simvastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the simple version: your body relies on a liver enzyme called CYP3A4 to clear simvastatin. In lab dish studies, uva ursi seems to slow that enzyme down. If that were to happen in real life, more simvastatin could linger in your system, which can nudge up the risk of statin side effects like muscle aches, weakness, or, rarely, more serious muscle problems.

I want to be honest about the strength of this, though. This signal comes only from test-tube work and has not been seen in people, so it's more theoretical than proven. Since both simvastatin and high-dose uva ursi can stress the liver, I'd keep uva ursi short-term at most and run it by your pharmacist or doctor first before pairing them.

Simvastatin also falls under GLUCURONIDATED DRUGS.

Theoretically, uva ursi may increase levels of drugs metabolized by glucuronidation.
In vitro, uva ursi extract appears to strongly inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1). However, uva ursi extract does not appear to inhibit UGT1A1 in animal models. This effect has not been reported in humans.

Here's the short version: this is a very theoretical concern. The worry is that uva ursi might block a liver pathway called glucuronidation, which could in turn let certain drugs build up. Simvastatin (Zocor) does undergo some glucuronidation, so on paper there could be slightly higher levels in your system. Higher statin levels can mean more of the usual side effects, like muscle aches, weakness, or rarely more serious muscle problems.

That said, I want to keep this in perspective. This effect has only shown up in lab tests, not in animals and not in real people. So I wouldn't lose sleep over it. Since uva ursi should only be used short-term anyway, just give your pharmacist or doctor a heads-up before combining them.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

About Simvastatin

What Is Simvastatin Used For?

Brand name: Zocor

Simvastatin tablets are used alongside a cholesterol-lowering diet to treat high LDL cholesterol (bad cholesterol), high triglycerides, and certain inherited cholesterol disorders, including heterozygous familial hypercholesterolemia (HeFH) in adults and children aged 10 and older, and homozygous familial hypercholesterolemia (HoFH) in adults.

In adults who already have coronary heart disease, cerebrovascular disease, peripheral vascular disease, or diabetes and are at high risk for heart events, simvastatin is also used to reduce the risk of death from heart disease, non-fatal heart attack, stroke, and the need for procedures to open blocked arteries. Zocor is one brand-name product containing simvastatin and carries these same indications.
