The piece to watch here is the pseudoephedrine in these combo cold and flu products. Pseudoephedrine is itself a stimulant that revs up your nervous system, and yerba mate adds its own dose of caffeine on top. Stacking two stimulants can pile on the jittery, wired feeling, so you may notice a racing or pounding heart, trouble sleeping, restlessness, headache, or higher blood pressure.

This is mostly a theoretical, common-sense concern (it's based on expert opinion, not hard studies), and a single warm cup probably won't cause much. But several strong mates plus a full-strength decongestant could add up. The acetaminophen and dextromethorphan parts aren't really the issue. If you tend to feel jittery or have high blood pressure, go easy on the mate while you're sick, and check with your pharmacist.

Acetaminophen, Dextromethorphan, Pseudoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

This combo cold and flu product has three active ingredients, and the one most affected here is dextromethorphan (the cough suppressant), which is partly broken down by the liver enzyme CYP3A4. In lab dish studies, yerba mate seems to slow that enzyme down a bit. In theory, that could let a little more dextromethorphan build up, which might mean more drowsiness, dizziness, or feeling foggy.

Honestly, this is a minor, theory-based concern. It comes from test-tube research, not real people, and a cup or two of mate is unlikely to cause trouble. Keep in mind the pseudoephedrine and caffeine in mate can both be stimulating, so you may notice extra jitteriness or a faster heartbeat. If you take this regularly, just check with your pharmacist.
