ADO-Trastuzumab Emtansine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yerba Mate interacts with.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Kadcyla (ado-trastuzumab emtansine) and yerba mate. The cancer-killing payload on this antibody drug, a chemo agent called DM1, is partly handled by a liver enzyme known as CYP3A4. In lab dish studies, yerba mate appears to slow that enzyme down. In theory, slowing it could let a bit more of the active drug linger in your body, which might nudge up side effects like low blood counts, fatigue, nausea, liver irritation, or nerve tingling.
That said, this is only test-tube evidence and rated minor, so casually enjoying mate may not matter much in real life. Still, since this is a serious chemo medicine, please run your mate habit by your oncology team or pharmacist before relying on it. Don't change anything on your own.

Kadcyla is used by itself (as a single agent) for HER2-positive breast cancer in two situations. Your tumor must test positive for HER2 using an FDA-authorized test.
- Metastatic breast cancer: for people who already received trastuzumab and a taxane, and who either had treatment for metastatic disease or whose cancer returned during or within six months of finishing adjuvant (after-surgery) therapy.
- Early breast cancer: as adjuvant treatment for people who still have residual invasive disease after neoadjuvant (pre-surgery) taxane and trastuzumab-based treatment.
